The ambition of the Knowledge Management Hub (KMH) is to maximise the value of the existing evidence and experience in integrated care in order to improve the capacity of countries, regions and organisations to adopt and scale up integrated care by:

  • Facilitating understanding of local readiness, needs and priorities for the adoption and scaling-up of integrated care by using the SCIROCCO Exchange online self-assessment tool.
  • Improving access to readily available evidence on integrated care.
  • Co-designing personalised support for those seeking support to prepare the ground for transition and/or improve existing system design for integrated care by facilitating tailored knowledge transfer activities.
  • Utilising outcomes of knowledge transfer and capacity-building activities to co-design improvement planning that is tailored to the local context, maturity and aspirations of European regions and organisations.

What is the SCIROCCO Exchange Self-Assessment Tool?

Maturity Model

The SCIROCCO Exchange self-assessment tool is an online self- assessment tool with an objective to assess a region’s readiness for integrated care. It builds on the conceptual Maturity Model for Integrated Care developed by the B3 Action Group on Integrated Care of the European Innovation Partnership on Active and Healthy Ageing. The Model has been further validated and tested through the EU Health funded projects SCIROCCO and SCIROCCO Exchange.

The SCIROCCO tool helps regions to:

  • Understand the strengths and weaknesses of their local context for integrated care and inform national, regional and local policy-makers about potential areas of improvement;
  • Facilitate multi-stakeholder dialogues on progress towards the implementation and delivery of integrated care;
  • Facilitate twinning and coaching activities that help regions and organisations to better understand the local conditions that enable the successful deployment of integrated care.

How can the SCIROCCO self-assessment tool be used?

The SCIROCCO Exchange self-assessment tool consists of different elements:
  • An on-line maturity assessment questionnaire:
    • organised along 12 domains of SCIROCCO Exchange Maturity Model for integrated care and the assessment scale
    • spider diagram illustrating the assessment outcomes
    • opportunity to share and compare spider diagrams of different users as a basis for consensus-building
    • available in 9 languages: Czech, Dutch, German, Italian, Lithuanian, Polish, Slovak, Slovenian, Spanish.
  • A set of methodologies on how to use SCIROCCO Exchange tool for:
  • A set of illustrative videos and materials on how to use SCIROCCO Exchange self-assessment tool
 

The SCIROCCO Exchange tool has been tested and applied in more than 75 regions and organisations in Europe and beyond, including Australia, New Zealand, Singapore, Canada and the US.
